Course Description
Diabetes Road Map  is a model to help nursing staff understand, communicate and implement their role in the support and delivery of care to the member/patient with diabetes across the continuum of care. This short course includes a toolkit designed to assist nurses to implement this model.

Identifies the physiology, standards of care, and ABC targets for Type 2 diabetes as well as reviewing treatment for hypoglycemia, patient scenarios, and skills to assist diabetes members.
