Program Description:

This program reflects Kaiser Permanente NCAL Region policies and procedures relevant to anticoagulation therapy. It will focus on the ordering process, clinical indications, monitoring parameters, and teaching plan for patients receiving any of the following anticoagulants:

  • Fondaparnux (Arixtra)

  • Enoxaparin (Lovenox)

  • Heparin

  • Warfarin (Coumadin)

Learning Objectives:

At the end of this presentation, participants will be able to:

  1. Describe the ordering process for the following anticoagulants:

    1. Fondaparnux (Arixtra)

    2. Enoxaparin (Lovenox)

    3. Heparin

    4. Warfarin (Coumadin)

  2. Identify who manages the daily dose adjustments for anticoagulants.

  3. Describe the clinical indications, adverse reactions, and monitoring parameters for specific anticoagulants.

  4. List the laboratory values that need to be monitored on patients receiving anticoagulant therapy.

  5. Describe the appropriate teaching plan for patients receiving anticoagulation therapy.

Continuing Education credit:

No continuing education credits will be awarded for this program. Please fill out and print a copy of the survey. Copy on the printed survey your responses to the questions. Click on the "SUBMIT" button, and give the printed copy to your Nurse Educator or Manager to document your completion of this program.
